Campers must be ages 7-14 years and must be deaf or hard of hearing.

The Mike Bush Fantasy Baseball Camp offers children with a hearing loss the opportunity to play the game of baseball during a week long summer camp.  Every reasonable effort is made to ensure safety for all children during the camp.  However, in the unlikely event an injury would occur during the week of the camp, the National Fantasy Baseball Camp, City of St. Peters, camp volunteers, and camp staff are not liable for compensatory damages as a result of such an injury.   To participate in the Fantasy Camp, each parent must show proof of insurance by completing this Liability Release Form.

I hereby release the Mike Bush Fantasy Baseball Camp of any and all liability for my child and will not hold the National Fantasy Baseball Camp, City of St. Peters, camp volunteers, or camp staff responsible for damages in the event of any injury, or loss of items during the week of the camp.  Finally, I give the Mike Bush Fantasy Baseball Camp permission to photograph and publish photographs of my child.
